Ensuring that residents’ wishes are honored at the end of life is one of the greatest responsibilities for an assisted living provider. Accomplishing this can be challenging due to a variety of factors, including regulatory conflicts, communication issues, and unfortunately a lack of education. The POLST, or Physician Orders for Life Sustaining Treatment form was developed to tackle these challenges by turning treatment wishes of an individual into actionable medical orders and by creating a document that is portable from one care setting to another. Ten states currently have POLST program in place, and numerous others are in active development.
