Are You Tracking Hospital Readmission Rates?

Starting in October last year, hospitals are being penalized by the Center Medicare and Medicaid Services for excessive hospital readmission rates. The penalties are intended to encourage hospitals to coordinate care of their patients and reduce the negative outcomes and costs associated with unnecessary readmissions. This means assisted living providers are having to pay closer attention to their own readmissions if they want to be looked at favorably by hospital discharge planners and clinicians.

Fortunately the National Center for Assisted Living has published a tool to help you calculate readmission rates for your residents using a formula that is consistent with other national standards.
